Application for 2009 USAC Point .25 Family Competition License & Parental Consent Form

1. Fill out application and parental consent forms
   (one parental consent form for each child)

 
2. jpeg headshots of all drivers and handlers pictures can be e-mailed to Mac-O-Chee Secretary or on a cd and mailed with application.
   (don't forget, you can have one free alternate handler with your USAC family membership)

 
3. Copies of birth certificates for all drivers

 
4. Make a copy for of all documents yourself

 
5. Send application & fees to Mac-O-Chee Secretary

Family - $75USAC + $50 Mac-O-Chee (voting rights as a family - Free Alternate

Handler votes with family; if Free Alternate Handler wishes to individually vote they must pay $50 club fee)

Associate/Additional Alternate Handler - $25USAC + 50 Mac-O-Chee (voting rights)

Associate/Additional Alternate Handler - $25USAC + $15 Mac-O-Chee (no voting rights)
